Individualized services and supports that enable a beneficiary to live successfully in a Group Home or Alternative Family Living setting of their choice and be an active participant in the community. The service helps increase or maintain life skills, provide supervision, maximize self-sufficiency, increase self-determination, and ensure full membership in the community.

Periodic support and temporary relief to the primary caregiver(s) from the responsibility and stress of caring for an eligible child, adolescent, or adult beneficiary. Respite may include in-home or out-of-home services, including overnight, weekends, or emergency care.

Individualized day activities that support the beneficiary’s definition of a meaningful day in an integrated community setting with persons who are not disabled. Services are designed to promote community participation, meaningful relationships, natural supports, independence, and valued social roles.

A service to support successful community living in unlicensed residences that are owned or leased by the person. It increases and maintains life skills, ensures health and safety through a supervision/staffing plan, supports self-sufficiency and self-determination, and ensures full membership in the community.

An individualized or group service that enables the beneficiary to live successfully in their own home, the home of family or natural supports, and be an active member of the community. A paraprofessional helps the beneficiary learn new skills and supports individualized activities aligned with the beneficiary’s preferences.

Residential Supports may be provided in an AFL situation where the site is the primary residence of the AFL provider, who receives reimbursement for the cost of care. These sites may be licensed or unlicensed in accordance with state rule.

Services that assist a beneficiary with I/DD or TBI, age 16 or older, with choosing, acquiring, and maintaining a job. The service is available when competitive integrated employment has not been achieved or has been interrupted or intermittent, and may be temporary or long-term.

Training for families and the beneficiary’s natural support network to enhance decision-making, provide education on I/DD, teach interventions and strategies, and strengthen the natural support network’s ability to care for and support the beneficiary.
